Straight Talk at Hurley Medical Center

Straight Talk Referral Process:

  1. Identify at risk youth. Appropriate referral would be a youth, age 8-17 that are at risk for, or exhibit high-risk behavior associated with fire play, fire setting or juvenile arson.
  2. All referring agencies must complete a Referral Form for each participant. Local Schools, Fire Department, Police Departments and Social Service Agencies should additionally complete a Waiver Disclosure Form. Referral Forms and Waivers should be faxed to: (810) 762-6341.
  3. Discuss Straight Talk referral with custodial parent or legal guardian and provide each family with the Straight Talk Family Information Forms (PDF).
  4. Inform the parent/legal guardian it is their responsibility to call and schedule their child/children into the Straight Talk Program within one week. Number to call for scheduling is (810) 257-9462.
  5. Inform the parent/legal guardian it is their responsibility to return the Straight Talk Program Certificate of Completion to your agency immediately upon attending the Straight Talk Program.
  6. Referring agency will be contacted immediately if youth does not attend the Straight Talk Program.
